FEES

   The fee for a basic, uncomplicated 1st, 2nd,3rd Class Physical is $120.00. ECG`s are 40.00. Additional fees may apply and can be supplied on request. 

Payment is due at the time of service. A fee may be charged if you fail to show up for the appointment without notifying us prior to the appointment time. We accept cash, personal check, Debit card, Visa, Mastercard, Discover, and American Express. We do not participate with any insurance programs and do not accept insurance assignment. We can supply a receipt, which you may submit to your insurance for possible reimbursement.

Additional fees may apply if contact with your physician or F.A.A. is needed and/or review of additional documents outside of those available during the exam or extensive records review.

Ask for the CFI, veteran or first responder discount. 

 

CACI (Conditions AME Can Issue) review of documents start at $25.00 per condition depending on complexity and volume of information viewed, (Cardiac cases starting at $75.00 due to their complicated nature).

Special Issuance including the AASI (AME Assisted Special Issuance) review of documents starting at $50.00 per condition. (Cardiac cases starting at $75.00 due to their complicated nature).

Consultation in the office or by phone for additional review of records or contact with the FAA on your behalf is billed in 15 minute increments, based on $200.00/hour.
